Abstract
Bone grafts are an integral and important aspect of craniofacial reconstruction. Ribs, tibia, and iliac bone have traditionally used as donor site but each of these has various problems, however using the calvarial bone as a donor site has several advantages. These are as follows; much less donor site morbidity & pain, only minimal resorption, easy to reach the donor site through direct scalp incision, less functional inability, shorter hospitalization, no need of immobilization, hidden scar by saclp hair, no secondary deformity and appropriate curvature obtained properly selected.

From March 1987 to October 1988, we experienced eleven cases of autogenous calvarial bone (outer table) graft such as tumors on the craniofacial area, facial asymmetry due to severe facial bone fracture, and cosmetic problems of nose (short columella, deviated nose, and secondary deformity of repaired cleft lip-nose). The results were very excellent without any significant complications except dura tearing and minimal bone resorption.

The detailed technique of autogenous calvarial bone graft and its advantges compared with traditional methods of bone grafts are discussed.
